If you have an appointment at Dr. Plucknett’s office please come prepared with information about what has been happening with you.

Getting Ready For Your Appointment

This means making sure we have your medical history, reason for referral (if you are referred from another Physician) and what you hope to have treated. This may include something called a “voiding diary” if you are being treated for incontinence. A good history of your problem will always help us to be better prepared to treat you.

And of course, we do ask that you bring your medical insurance card, proof of identification and be prepared to pay for your co-pay at each visit.  Should you have any financial obligations or outstanding balances for previous medical care that we provided for you, we will ask that you settle all financial accounts in a timely manner. We do understand that these are difficult economic times and we are open to speak with you privately if you have financial concerns related to paying for your medical expenses.

Appointment Cancellations

There is a no-show fee for appointments cancelled with less than 24 hours notice. That fee is $75 for established patients and $150 for new patients.
